PURPOSE OF THE UNIT STANDARD
This unit standard requires learners to demonstrate the ability to conduct specialist investigations into workplace incidents.
Learners credited with this unit standard are able to:
- • Explain the theory and principles of incident investigation.
- • Explain the initial response required at an incident scene.
- • Investigate the incident, gather evidence and describe the review/follow-up processes.

SPECIFIC OUTCOME 1
Explain the theory and principles of incident investigation.
ASSESSMENT CRITERIA
ASSESSMENT CRITERION 1
Losses in terms of financial, social, emotional and environmental impacts are explained with regard to incident investigation processes.
ASSESSMENT CRITERION 2
Key functions of respective team members during the investigation process are explained.
ASSESSMENT CRITERION 3
Specialist incident investigation techniques required at a specific incident are identified and explained.
SPECIFIC OUTCOME 2
Explain the initial responses required at an incident scene.
ASSESSMENT CRITERIA
ASSESSMENT CRITERION 1
The need to preserve evidence at the scene of an incident is explained and the requirements for achieving such preservation are demonstrated.
ASSESSMENT CRITERION 2
Specific emergency responses to an incident scene are described with regard to restricting injuries and to containing damage.
ASSESSMENT CRITERION 3
Processes to activate mutual aid arrangements are applied to limit further injury to people and damage to the site.
SPECIFIC OUTCOME 3
Investigate the incident and describe the review/follow-up process.

Evidence collected in terms of sources and data-gathering methods are applicable to the specific incident.

All collected data are collated and analysed to provide evidence to the investigators.

Recommendations are formulated and forwarded to the relevant authorities.
ASSESSMENT CRITERION 5
Incident documentation is revised within the specified time.
ASSESSMENT CRITERION 6
The investigation recommendations are monitored to ensure that they are applied.
UNIT STANDARD ACCREDITATION AND MODERATION OPTIONS
- • Anyone assessing a learner against this unit standard must be registered as an assessor with the relevant ETQA or ETQA that has a Memorandum of Understanding in place with the relevant ETQA.
- • Any institution offering learning that will enable achievement of this unit standard must be accredited as a provider through the relevant ETQA or ETQA that has a Memorandum of Understanding in place with the relevant ETQA.
- • Moderation of assessment will be overseen by the relevant ETQA or ETQA that has a Memorandum of Understanding in place with the relevant ETQA according the moderation guidelines in the relevant qualification and the agreed ETQA procedures.

• When investigating incidents, the individual will have to collect and organise information in such a way that he/she will be able to evaluate it and make decisions.

Communicate effectively using visual, mathematics and language skills in the modes of oral and written presentations.
• Remedial action resulting from the investigation must be communicated to all relevant persons.
